We now offer the facility to our patients to order their routine repeat
prescriptions by email. The following conditions must be satisfied though:

All orders must be via this form. Please don't send email via another route
as this form ensures that we get all the information we need and should the
service be modified or suspended then there will be a notice on this page.

Only authorised repeat medications with at least one unused repeat left may
be ordered on this form.

The form requires you to enter a return email address for queries and you
should check your email one working day after posting the request in case
there are problems with your request.

Allow two working days for the prescription to be printed off and signed.

Normally prescriptions should be collected by yourself, a representative
or the chemist. There is a space to nominate this individual by name - if
it is left blank it will be assumed that you will be calling personally for
it. We will only mail the prescription out if you have already lodged a stamped
and addressed envelope with us.

For confidentiality you may choose to identify youself using the number to
the left of your name on the prescription counterfoil rather than your name
(email should not be regarded as wholly secure). This also avoids confusion
if another patient has the same name as you. (Don't use any other numbers
as they may refer to our old computer system). If we can't identify you we
will email you back for clarification with all the delays that that will
cause.

If you receive the same drug in more than one dose or formulation make sure
that your request makes it clear which you are ordering. Again any lack of
clarity may result in our having to email you back.

Any special comments such as the need to order extra early before a holiday
please put in the comments box. If you need double quantities for any reason
put the request and the reason here.

Order early rather than late.
We have provided on the form sufficient space for you to order four items.
If you need more then send more than one form please.
If you have a query about a drug or wish something different or if your
repeats have expired you should contact the surgery in the normal way. There
is a facility at the Virtual Surgery
to ask questions about your medication but you must allow a week to receive
a reply.
If you wish all your items to be issued enter in the
FIRST ITEM field "all" followed by the total number of items in brackets
- to serve as a check. e.g. "all(6)". However, if the number does not tally
with what we have on record we will have to email you back causing delay.
